What exactly are you trying to accomplish? Are you trying to create a pdf or
extract information from a scanned form? If you're trying to create a pdf
and you're only talking about a handful of forms, it's going to be easier to
create them using something like Acrobat Professional. If you really want to
create a pdf from a scanned image and/or you want to extract information
from a scanned form that's had information entered on a it, you need to
research OCR forms processing, like A2iA or Mitek Systems products. I don't
think any product will automatically create the pdf, it will only help you
extract information to build some sort of template from. You could then use
iText to create a pdf from the template. Would be very challanging.
