CMYK is fully supported for active fields - no need to flatten.

However, if you do wish to use Spots, then you have no choice but to flatten...

I'm developing an app that allows users to modify the acrofields in an
existing pdf. But now I need these fields to support CMYK and spot colors. I
think the only solution is to flatten these fields, but I don't know how to
change the colour.
