Then you will need to add the necessary tags/structure to the image when you 
put it into the PDF via iText.

There are no high level APIs to do this - you will need to work at the lowest 
level...

1. When using direct content under:
- the image is present in the tags tree (View --> Navigation panels --> Tags), 
and it is present in the order and content trees.
- image is placed on top in the order tree, regardless of its placing on the 
page (which is a separate problem).

2. When using Document --> Background --> Add/Replace.. from within Adobe 
Acrobat Pro 9 (or alternatively, View --> Navigation panels --> Order and place 
the image in the background using right-click and then selecting "Tag as 
background"):
- the image is no longer present in the order and tags trees, only in the 
content tree, and there it is placed in a "Container <Artifact>".

This second case is the behaviour that I want to achieve using IText. The 
customer demands that the image has an alternate text and it is placed in the 
background.
