I'm not sure if I understand this.
If you make the lly component in the Mediabox negative (lower the lower 
left Y-coordinate), then you create extra space at the bottom. The same 
goes for the llx coordinate: you create extra space on the left.

In that case, it's very easy to achieve using PdfImportedPage.

The pages of one PDF are added to another PDF as if they were images
(but as I said before: you'll lose all interactive features; dealing
with that interactive content is not obvious).

If it were Java, I'd try running it, but I'm not going to translate
your example to Java on my own time.
