One question:
Are you opening the template (not the resulting PDF) in Adobe Designer?
If your answer is Yes than this behavior (empty text fields) is absolute
normal.
If you want to make some changes on the template, then you can use the
"Default Value" of the text field properties to add some text manually and
see how the text field behaves.
