Nothing is wrong with the form.
It's just that you opened the PDF in Adobe Reader which is a product 
that you can use to view PDFs. You have submitted the form to a server, 
and the server returned HTML in answer to your request.

Now Adobe Reader says: "hey, why are you sending me HTML? I'm a tool 
that can be used to view PDFs. Why aren't you using a browser to view 
your PDF? At least open the form in the Adobe Reader browser plug-in and 
don't bother the nice people on the iText mailing list with silly 
questions like this." (That's more or less what the error message means.)
