I tried to duplicate your PDF (a link/attachment would have been Quite Handy). I ended up using an XML Form. I added a field with a MM DD YYYY format. The resulting PDF also did nothing to enforce the formatting. I could find nothing in the resulting PDF that corresponded to the formatting I selected. No JavaScript, no XML, nada. I suspect this is a "todo"... that OOo isn't doing anything with the data in PDF. OTOH, you could be using a completely different method to generate a PDF form, which may or may not provide Useful Information to the PDF.
