I fear the concept of "baseline" in PDF Forms is problematic. It has 
been changed over different versions of Adobe Acrobat/Reader. I don't 
think there's a "ready made" formula that will give you this offset; and 
there's no way to change the offset of content in an AcroForm field.

You'll have to use trial and error; and choose the solution that matches 
the final result as close as possible.
