There is a bug in iText 2.1.5 (try iText 2.1.4 or wait for iText 2.1.6).

However, image.setAlt() isn't the way to achieve what you want.

Unfortunately, due to changes with respect to Tagged PDF in 2.1.5;
this functionality was broken. The changes have been undone and
starting with 2.1.6, the example will work as expected.
