I have had excellent success using JPedal for PDF conversion
to Graphics2D.

I also had the opportunity to work with their support team
on a buggy PDF and they fixed the bug and sent me an updated
binary.

This was all for the FREE version.  I look forward to being
able to purchase the full version.  They seem to be a 
good company to work with.

No, those are parameters specific for Adobe Reader (see Appendix C of 
"iText in Action").

PDF isn't PostScript. There are printers able to print PDF directly,
but you shouldn't count on that. Printing is discussed briefly in
chapter 18 of the book, but I think the safest way to do it in Java,
is to use a library/product that renders the PDF.

In the book, I mention JPedal (IDR Solutions).
Meanwhile there's also PDFRenderer (SUN).
