Do you want to rewrite the pdf so that the comments are displayed by default or
just literally render it and present a series of pictures to the reader. In the
latter case, you
may in fact be able to just use an open source renderer
and save the rendered page bitmaps in your favorte format that allows them to
be incorporated into de novo
pdf file.
