You'll find a similar problem explained in the second edition of the book. There's a datasheet form with fields title, director,... Then a new form is created containing more than 1 datasheet page. As you pointed out: you risk ending up with duplicate names, and that doesn't work. You can have duplicate field names in a PDF form, but fields with the same name, should also have the same content (and that may not be what you want).
