You'll find a similar problem explained in the second edition of the 
book. There's a datasheet form with fields title, director,...

Then a new form is created containing more than 1 datasheet page.

As you pointed out: you risk ending up with duplicate names, and that 
doesn't work. You can have duplicate field names in a PDF form, but 
fields with the same name, should also have the same content (and that 
may not be what you want).

In the book example the fields are renamed to movie_1, director_1,...
movie_2, director_2,... using this code snippet:

Once the fields are renamed, you can concatenate the forms.
