Sounds like you have a Reader Enabled form.
Filling such a form the wrong way causes this problem:

"This document enabled extended features in Adobe Reader. The document 
has been changed since it was created and use of extended features is no 
longer available. Please contact the author for the original version of 
this document."

Section 8.6 of "iText in Action - Second Edition", entitled "Preserving 
the usage rights of reader enabled forms" explains how to fill out a 
reader enabled form without breaking reader enabling.
