Thanks for the responses. The setting Edit > Preferences > General > uncheck "Create links from URLs" only affects "incidental" links, not links specifically created in the document. Turning that off stops Reader from creating links out of flat text that wasn't an explicit link to begin with, but it has no effect on links explicitly created with visible text like "Click Here!" and a specific URL.
There certainly IS a way to control this in HTML. The 'title' attribute of the A tag controls what will show in the tooltip. For no tooltip, omit the 'title' attribute. I looked in the PDF spec and did not see any provision for something analogous to the 'title' attribute on a hyperlink.
