You can't create a PDF, nor a PDF template with "Acrobat reader". 
"Acrobat reader" used to be the name for what is now known as "Adobe 
Reader". You are probably referring to "Adobe Acrobat".

With "Adobe Acrobat" you can create PDF forms that are based on AcroForm 
technology, as opposed to PDF forms created with Adobe LiveCycle 
Designer that are based on the XML Forms Architecture (XFA).

There are many different ways to create a PDF form based on AcroForm 
technology. For instance: in chapter 6 of the second edition of "iText 
in Action," you can learn how to create a PDF form using OpenOffice.org.
