I would love to do that, especially since PdfStamper seems to be the only
class that can flatten the annotations, unless I am missing something (which
is likely).  However, I need to be able to save the file as PDF_VERSION_1_4. 
If I can use the writer from the PdfStamper to do this, then I think I
should be able to switch....  But I need to know the correct process for
flattening the annotations.  I can only find articles on flattening form
fields...

Yes, that is correct.  The PDFSharp library has a limitation that it cannot
handle PDF files that use what it labels as "iRef streams" (I assume that
these are the same things as the "compressed XRef tables" that you mention).  
Consequently, only older Acrobat 5 PDF files will work completely with
PDFSharp.\
