The dictionary values are correct, there is no XMP, not got there yet.

 

Attached doc5.pdf which shows A,B,C,D in the info dialog and different

I'm not sure.
What if you look at the PDF using iText RUPS (there's an exe now; see
http://sourceforge.net/projects/itextrups/ ).
Is the metadata in the Info dictionary consistent with what you expect?
Is the metadata in the XMP stream (if present) consistent with that
metadata?
