If you're flattening, be sure to throw away the XFA!

OK, then we've established that you do have AcroForm fields.
You also throw away all interactivity, because you're using 
setFormFlattening(true); What happens if you open the PDF that is filled out in 
Adobe Reader?
Does it look OK? Are the fields filled in?
