I can see the overlay (it's added as /Xi6, object number 1 in the PDF), 
but it's not in the "visible area".
This is how you added it: q 1 0 0 1 0 0 cm /Xi6 Do Q
You've used coordinate 0,0, but in this case you should have used 0,12407.1.

No errors are produced because the overlay is added where you told iText 
to add it.

Nothing wrong with those files. 3_overlay.pdf is added to base.pdf, but 
not visible in output3.pdf because you've added it outside the visible area.

We want to help you help yourself. Please read the book p145: "When you 
add content at an absolute position, you'll need to take the (x,y) value 
of the origin into account if it's different from (0,0). Otherwise, you 
risk adding content in the wrong place, maybe even outside the visible 
area of the page."
