Sometimes I even surprise myself :-)

Let me finish coding up my "workaround" and I'll post it.  Maybe it will
be helpful to others...

Not for the moment. You're not "missing" something.
It's just that iText could do better in this case.

I think you fully understand the issue: you understand that Document and
PdfDocument are different.
The margins in Document are whatever you tell them to be, as soon as you
change them.
The margins in PdfDocument are whatever they have to be. If you change
them, they change on the next page.
Your workaround is OK.
