In that case, the problem should be clear: if you want to use a PDF A as stationery for a PDF B, it goes without saying that the page sizes of both PDFs should be equal. If they are not equal, it shouldn't surprise you that header or footer disappear. Also: if PDF B is opaque, adding stationery under the existing content, you shouldn't expect to see the header/footer.
