The problem is caused by a tool (I won't name it because I don't want to make bad publicity for competitors) that creates crappy PDFs. The "iText in Action" book comes with an example that can be used to fix forms that are broken like this: http://itextpdf.com/examples/iia.php?id=236
Plenty of the examples in the book are actually answers to questions that were once posted to the mailing list ;-)
