As I have neither your smartcard nor your middleware, I cannot reproduce the result of running your code. Therefore, please also supply a sample document signed by your code which we then can inspect. Looking at your code you seem to work with a pre-5.3.0 version of iText. As you seem to be early in your development, you might want to consider updating and using the tidied up iText security API.
