As Bruno already explained, you need to give enough information to reproduce the issue. This includes the PDF in question. BUT at least one thing is immediately visible: You use an old iText version. Many bugs have been fixed and many improvements have been made in the 5.x versions in the context of parsing texts and fonts in existing documents. The state of that code in the pre-5.x versions should be considered merely a proof-of-concept but not ready for production use.
