First of all: the "Annotation modified" message is no ERROR, merely a WARNING, at least here Adobe Reader merely shows a warning sign. Now concerning the warning: As the revision with the signature indeed is the outermost revision in the file, the warning is due to SOMETHING ADOBE READER DOES while displaying, not due to a after-signing-change in the file itself. And indeed, when you create the freetext annotation, you do that without adding an appearance stream. Thus, Adobe Reader has to create an appearance for that annotation. And as Adobe Reader in its signature panel tries to describe the document you see, this appearance it created constitutes a change of the annotation after the signature was applied. And this exactly is what it warns about. Up to a point it is right to do that: After all, different PDF viewers may create appearances for that annotation differently, at least the exact positioning may differ, and that MIGHT make a difference. To get rid of the warning, therefore, you should create an appearance for your annotation when you add it.
