Since originally posting i've got a few more details:
It happens after entering and exiting back to the menu a few times. Be
that watching live tv, video or recording. Sometimes I can exit a few
times before it breaks, sometimes it'll do it after only one.
I've since discovered restarting X solves the problem, so a reboot is
not neccessary.
All rpms are from atrpms, both mythtv and ivtv .
Once it has broken, even running a video in mplayer from the command
line doesnt work.
I've attached the output from xvinfo after it has broken, and also the
error produced by mplayer

autopaint overlay setting but for some reason is getting something wrong in
the way it sets it, hence these messages.
I would be surprised if this is an xdriver problem so it is more likely to
be myth or something broken on your system.
I can't see anything in myth either though and I don't remember seeing any
changes in that area of the code so I would be suspicious of something else
breaking.
Did you build myth from source or download it from somewhere? I wonder if
there is a possible Xv/shared memory incompatibility between X and the
libaries used to build myth for some reason

As you can see, I have only one screen configured.
As for the Xdriver:

What is the output of xvinfo?
Are you definitely using the latest Xdriver?
How is your xserver configured (single screen or multiple
screens/cards and if so what are they)?

Since upgrading to 0.19, live tv will randomly stop working.
The sound will play fine, and i can change channel etc,
