I feel your pain.  I had similar lockups using the PVR-350's decoder.  I
could run with no problem when using my onboard nvidia to output to the
monitor.  But whenever I tried to use the TV-OUT of the 350 I would get a
hard lockup usually within one or two hours of watching live TV.  I tried
everything for about two weeks but could not get it working.

By chance, I was swapping the DVD drive on my myth box and left the case
cover off.  I let the PVR-350 run for a couple of hours with no lockup. 
Within an hour after replacing the cover my box locked up again.  I started
to consider overheating as a strong possibility.  Googling around I found
that the 350 decoder runs very hot and can cause the same type of lockups
when using the WinTV2000 application in WinXP.  So I installed a slot fan
next to the 350 which pulls air directly off the decoder and out the back
of the box.  I haven't had a lockup in over 24 hours of watching live tv
since.

I have a low-profile htpc case (Silverstone LC11) which uses a PCI riser
card to install the cards horizontally.  I originally had the 350 in the
lower slot which had it placed very close  to (almost touching) the
motherboard.  To make matters worse, the expansion slots are very close to
the power supply.  I could feel that this area of the case was warm to the
touch when I had the decoder running.  Now with the slot fan, it is much
cooler.

Anyway, this may not be your problem, but you may want to try cooling your
350 down and see if anything changes.
