Try:

modprobe tveeprom
modprobe cx25840
modprobe ivtv

Some folks have reported some possible timing issues, might want to give it a
moment or two in between modprobes.

There was something slightly off in tuner.c of the megapatch. I highly suggest
trying 0.3.2f, the only thing missing is the slightly updated tuner def for
the FQ1236A MK4, but my tuner worked with the older one pretty well also. I
submitted another patch against 0.3.2f that fixes that tuner def. There's
also a patch against f that should let the driver work on stock FC 4kstacks
kernels, which I hope to try out before much longer (maybe tonight, finally
got an FC3 test box up and running).
