As a follow up, my card, which now has this in the startup: cx25840: FW image '/lib/modules/HcwMakoA.ROM' of size 13353 loaded. cx25840: FW image md5 digest: aa4d1e3dd17667957d8fe5b8d2d19395 cx25840: FW image sha1 digest: 60eafed58e943664aae4754b32271e09a95496b4
works just fine. I got the above firmware using apt-get install ivtv-firmware-audio on a FC3 box (even though it didn't have a PVR150 card in it) and ftp'ed it to the box that has the card.
Only other change is to add alias char-major-81 videodev alias char-major-81-0 ivtv options cx25840 no_black_magic=1
to /etc/modules.d/ivtv on Gentoo and run modules-update
