Have you tried removing ivtv and reloading it again rather than just
Firing up myth after a reboot?

i.e.
#rmmod ivtv
#modprobe ivtv

Then starting mythbackend and mythfrontend.

It sometimes seems to work better/more reliably for some reason ...
