I had a KnoppMyth R5A10 with a PVR250 that worked very well until we
had a bad lighting storm and the PVR250 was unable to lock onto the
picture (The picture would wiggle and lose sync not matter what the
channel was).
So I assumed that some of the tuner hardware was fried and I went
looking for a new tuner card. I came across the PVR500 and decided to
take a chance since I had been considering getting a second 250
anyway.
Wow, I had a fried PVR250 as well, replaced with a PVR500 D492.

I think you're over doing it here. The tuner should auto-detect and
most of those options aren't necessary, and might be for a different
version of the same card. My D492 works just fine with :

alias char-major-81-1 ivtv
options ivtv tda9887=0,0 ivtv_debug=1,1
I don't have no_black_magic specified for cx25840, since I'm using a
newer ivtv build than you. I would recommend a build >= 0.3.4p,
though, as there was some good patches added to the cx25840 between
your version and the newer ones.
and to use the following ivtvctl lines.
/usr/local/bin/ivtvctl -d /dev/video0 -u 1 -f width=720,height=480 -
p 6
/usr/local/bin/ivtvctl -d /dev/video1 -u 1 -f width=720,height=480 -
p 6
Not necessary with Myth as long as you have the cards setup right and
your recording and livetv profiles all set to 720x480.
