Please check the wiki http://ivtv.writeme.ch/tiki-index.php?page=TvOutHowto to get started. The versions mentioned there are a little outdated but it is still valid in priciple (load ivtv-fb and have a X display associated with it). When you have that running you can reference the TV out display just as any other X display by setting the DISPLAY environment variable or giving "mplayer -display :0.1 ..." (i.e. if it is the second display on your first X server).
When you have this running you may want to check out recent versions of ivtv and John Harveys X driver to get better performance. Please note that the -vo xv option is supported only with the most recent drivers. You will have to use -vo x11 with all ivtv version up to ~0.3.5.
