I just wanted to follow up in case anyone on ivtv-devel is following this thread - over on the xen-devel list Keir came up with a fix for the problems caused when trying to use ivtv in a Xen domU. The fixes are in Xen 2.0-testing, which I assume will become 2.0.7 at some point (and should also get pushed into 3.0 when it comes out).
