Just use these options, that should be sufficient.

options cx25840 cardtype=1
options ivtv tuner=57

cardtype=1 means you have a pg600, 0 means a pvr150. I'm going to remove
this option in the next version, after all ivtv knows that it has a pg600,
so why would you have to specify it again?

If it still fails with these options, then let me know and I'll make it
easier to experiment with various input combinations.
