Welcome to my world of confusion! Looking through /var/log/messages this morning, I searched for 'pvr 150' and found it! I know that the card is not a second 250, because it is a different size. Further poking at /var/log/messages showed me one chunk from Monday night with a PVR-150 being initialized, and one entry from yesterday morning with two 250's being initialized (I was not awake at 4:00am, so the machine must have reset?). At least, I think that's what I see?
