I'm no expert, but it seems in all other ivtv dmesg output i've seen 
there is more info on it attaching to the tuner module...yours seems to 
be missing that info, perhaps thats why its not recognizing the 0x40 
commands, they are meant for the tuner which it is not seeing?

my own ivtv dmesg out has lines like these: (but then again my myth box 
is broken at the moment, so take it with a grain of salt :)

tuner 1-0061: chip found @ 0xc2 (ivtv i2c driver #0)
ivtv0: i2c attach to card #0 ok [client=(tuner unset), addr=61]
<snip>
tuner 1-0061: type set to 21 (Temic NTSC (4039 FR5))

that last "tuner 1-0061" line above is exactly where your 0x40 errors 
start in my output...I don't know what your problem could be, but you 
might want to focus on the tuner module.
