I know there is a similar thread called 'ivtv-0.6.2 not loading firmware.'
Essentially, I am having the same problem, but with 0.4.5. Apologies for the
long post.

WHAT I'VE TRIED SO FAR.
I am using 0.4.5 because I was having real difficulty with earlier driver
(0.4.2 is current Gentoo release)
Checked md5sums of the firmware files -- they match the How-to
Made sure I used an -el in 'vdl...'
modified FIRMWARE_DIR= in firmware.agent to point to hotplug/firmware directory
