I do exactly that - I have an NVidia Geforce2 (twinview, two CRTs) and
a PVR-350 using svideo out to a TV. I run two separate X sessions at
once on different VTs in xorg7. If you are using the mpeg output of
the PVR card, you can use the normal display x session at the same
time as the TV, including controlling it with the remote. If you use
framebuffer (ie, the menus), you will find that the screen does not
update unless it is the currently selected VT, so you need to switch
to it to use the fb menus (and the primary display on the CRTS goes
away during that).

There is a feature of xorg7 that allows multiple screens to be enabled
at the same time. Sharevts, I believe. It works, both screens are
active and work at the same time. However, it eats up about 100% of
CPU time, so you can't really use the PC for anything.

Solutions to that problem are welcome, but I just wanted to give you a
heads-up on what to expect when you try it
