Maybe a shot in the the dark, but have you tried the new ivtv 0.4.5 cvs . I see that a bugfix has been released for "Fix v4l2_frequency handling" . I see from your ivtv log this "ivtv0 warning: i2c client addr: 0x21 not found for command 0x8004646b!"
And if your kernel use udev you don't have to put in anything in your modules.conf because everything are beeing autodetected under boot. Have you shutdown your computer 30 seconds without power. The PVR-150 card needs sometime remove som old firmware.
