FWIW, I believe I've solved this problem (although I think there's still an underlying bug). I noticed that I never saw the problem when watching live TV under mythtv. I thought about it and remembered that when I setup my system originally, I made some changes to the "Default" profile that is used to record shows. Sure enough, the resolution was different from the "Live TV" profile. The live TV profile was 720x480, but the default profile was 640x480. I changed the resolution, and haven't seen the problem since.
