Just to follow up, this did turn out to be a bad PVR-350 card.  I had it
replaced and the new one worked fine.  Thanks for the reply!

I know of only one issue that may remotely cause this problem: the
saa7127 module has a problem with the saa7129 chip. So if the old card uses
a saa7127 and the new one a saa7129 then you need to use kernel
2.16.16.12 or up. Or apply the misc/saa7127.diff patch by hand.

Still, it's somewhat unlikely that this would completely hang the computer,
usually it just results in bad TV-out quality.
