The frame looks very similar (but not identical) to a video standards
settings mismatch between PAL and NTSC.  I would have expected the
colored lines to have gone all the way across the screen though.  Either
that or your CX25843 chips aren't getting set up properly by the cx25840
module for some reason.

Check that you're set to the correct video standard for the input with

$ v4l2-ctl -d /dev/video4 --log-status

(or /dev/video3 as appropriate.)

So you're implying it's unlikely to be the driver.  What about the
MythTV build or setup then?

For future reference, grep'ing on ivtv can leave out useful messages,
such as those related to the other modules: tuner, cx25840, wm8775, etc.
that don't have the string ivtv in the message.  It's best to provide
everything in between "ivtv: Start initialization" and "ivtv: End
initialization" like so:
