Hmmm. It looks like the device should be a PVR-150. But the lspci
output looks like the Ids might be incorrect. Could you please try
lspci -nnvv?

Try pulling out *all* your PCI cards, blowing the dust out of *all* the
slots, put them all back in, and try again.
