In our case the FAQ is managed by hand (using xooki as any other page).
Providing a TOC would be easy using a jquery plugin (for instance):

For checking that all faq entries have a unique anchor, following the xooki
philosophy we could do it with javascript. According to the frequency at which
we update the FAQ, I'm not sure it's worth the effort.
