Standardise the attribute names

The attribute "module" has been used in <info> and <exclude> to indicate a
module's name.
However, <dependency> uses attribute "name" to indicate a module's name.
There are also attribute "org" and "organisation" found in some of the Ivy tags.
We should standardise the attribute names.
