I had this problem ... the documentation I wrote about it is at:

which is all in terms of an RDF system Jena; we use Xerces as our XML parser.
Basically all our earlier code and documentation assumed the use of Reader's and Writer's which was simply wrong for Web applications.
The documentation was initially written to support our user base migrating to the fixed code.
