I'll put up a few possibilities. Some of them may be very obvious, but it's 
better to check anyway...

1. Is the program really there? Try to read it back from the PIC!

2. Is Power working? I'm measuring directly at the pins of the controller. 
(Pin5=GND, Pin14=5V)

3. If you haven't used pin RA6, and you have some means to detect a 1MHz 
signal, look for the clock when defining INTOSC_CLKOUT. You can use a LED, it 
should show that the pin RA6 has "both" Vdd and GND.

4. If Vdd rises at a very slow rate when switching on, the internal reset logic 
may not work. Vdd should rise from 0V to 5V within 100ms max. You can make it 
work even when Vdd is rising very slowly by defining "pragma target BROWNOUT 
enabled". (brownout works only when Vdd=5V, not at e.g. 3.3V) I would also 
recommend "pragma target PWRTE enabled" that will wait some milliseconds before 
starting the PIC, to let the power stabilize.
