I have written one, quite sophisticated and very easy to use. Multiple
recipients supported, user-definable tags supported etc.
You can download it from http://www.kbmail.com/FREE/KBmailForm.zip and
freely use it (sample & README included). Let me know you think.
The class is UTILS.KBmailForm.class so you should know what to do.

BTW: use at your own risk, and don't spam others by this poor stuff :-)
