I got this problem just last week, and it was because I had forgot to
include the line:

--activate-module=src/modules/jserv/libjserv.a

in my configuration of Apache.  Without this the Jserv configuration doesn't
seem to be run, servlets are not mapped to /servlets, and the servlet is
treat as regular document.
