Remove internal hostnames and project references from modernization plan

Replace penguin references with generic Apache httpd / localhost. Replace
RAPI-specific references with generic Axis2 JSON-RPC terminology. The
modernization plan is published on GitHub and should not contain internal
    infrastructure names.
